Kentucky adults are among the least healthy in the nation. In response, Foothills Community Action Partnership and Gateway Community Services Organization, both community action agencies, have collaborated to provide a unique healthcare service called The Healthy Communities Access Program (HCAP).
This program has been funded by the U.S. Department of Health and Human Services' Health Resources and Services Administration (HRSA) to promote and create healthy communities in Bath, Estill, Menifee, Morgan, and Powell counties.
HCAP addresses prevalent health deficits in Kentucky by focusing on obesity awareness and reduction. HCAP incorporates healthcare and professional training, client outreach and recruitment, physician and hospital accessibility, pharmacy assistance and healthcare consortium networking.
Foothills Community Action Partnership and Gateway Community Services Organization, along with other community agencies from the service area will make up the Foothills-Gateway Consortium to better coordinate these services, reduce duplication and provide referrals for the program. The GCAP consortium will work together to coordinate and strengthen health services. |
