This unique program will utilize tow mobile education vehicles to provide a full range of preventative health and wellness services. The target population suffers from numerous health risk factors in addition to obesity, including smoking, high-fat diets, high blood pressure and poverty.
Program activities will be designed to address nutrition throughout the life span by providing health education

• to the public,
• to professional service providers such as physicians and educators,
• and to non-professional service providers, including churches, other faith-based entities, community-based organization and social service agencies.

HCAP will also enhance healthcare for the uninsured and underinsured in these areas through benefits coordination and a voucher program. Dedicated county benefits coordinators will help access local, state and federal assistance programs to enroll clients who are eligible. A voucher program will help eligible participants who meet certain guidelines to access and pay for medical services, including dental and pharmacy services.
The program will provide services either directly or through established arrangements with providers without regard for ability to pay.
